Letter to members Equal Pay Flyer
STEP 1 – Check that your membership information is up to date. Visit https://my.unison.org.uk/ and log in using your membership number.
STEP 2 – Obtain a copy of your job description and your contract of employment. If you do not have these, you can ask HR or your line manager for a copy.
STEP 3 – Download and complete the case form. Fully complete section 1 and then complete your job title in section 2. Put a cross in the declarations in section 5.
STEP 4 – On the continuation sheet answer the four questions. Your claim cannot be processed if these are left blank, so please ensure that you complete this section.
STEP 5 – Email your completed form to Bradford.equalpay@unison.co.uk along with a copy of your contract/letter of appointment and your job description. If you can, put your name and membership number as the subject of the email.
