To make a claim please read the guidance notes and then download the case form and declaration. Once completed these should be returned to equalpay.yh@unison.co.uk along with a copy of your contract/letter of appointment and your job description.
If you do not have a copy of your contract/letter of appointment and job description, please request these from your line manager or HR.
